Parent Involvement:
We know Parent(s)/Guardian(s) are busy and it is difficult to always find time to talk, but daily communication helps us to better meet your needs and the needs of your child.
We meet individually with Parent(s)/Guardian(s) on a regular basis to discuss their child’s progress, share observations and work together to set individual goals and objectives for each child.
Day care Forms:
Before beginning at the day care the Parent(s)/Guardian(s) is required to complete the following specified daycare forms;
– Health inventory,
– Immunization certification,
– Emergency information card,
– Medication administration authorization (prescription and OTC),
– If applicable Court documents as to custody and visitation.
These forms must be updated as changes to the Parent(s)/Guardian(s) situation requires, including any changes of employment, contact information and telephone numbers, addresses and immunization updates.
Trial Period:
There is a one week trial period. There will be a regular week’s payment required at the beginning of the trial period. If either party is dissatisfied for any apparent reason before the one week trial period is over and both parties cannot reach an agreement, the contract will be voided and the service will terminate. There will be no refund for the trial period payment. After the trial period is over, two weeks notice for termination of the contract will be required.
Pickup Arrangements:
No one other than the Parent(s)/Guardian(s) listed on the emergency information card will be permitted to pick up your child. Advance written notice will be required of Parent(s)/Guardian(s) if someone else will be picking up the child. Please remember this is for the safety of your own child. If both the Parent(s)/Guardian(s) do not sign this agreement, documentation as to the legal and physical custody of the child must be provided.
Substitute Care Arrangement:
Parent(s)/Guardian(s) must have arrangements for substitute care in the event that the provider is unable to care for the child. The Parent(s)/Guardian(s), not the Center, is responsible for arranging substitute care.
Appointments & Early Pick up:
Parent(s)/Guardian(s) must notify us in the morning if they will be picking up the child early that day, or if the child has an appointment and will be leaving and returning again later in that day.
Hours of Operation:
Open Monday – Friday 8:30 a.m. – 4:30 p.m.
Closed on Saturday and Sunday.
Guidelines for Releasing Children:
The Center will release the child only to the Parent(s)/Guardian(s) with legal custody or to anyone the Custodial Parent has authorized only by prior arrangement with the Center in writing, or to the Police or Welfare Workers with proper authorization.
Food Safety:
Please provide SAFE FOODS for your child to make mealtimes safe and to prevent childhood choking,
Avoid sending the following foods in your child’s lunch:
Raw vegetables such as Carrots, Cherry Tomatoes,
Unpeeled fruit slices of Apples, Pears, Grapes,
Died fruit such as Raisins or Apricots,
Hotdogs or sausages served whole,
Cheese cubes,
Spoonful of Peanut butter,
Snack chips.
Food Allergy & Other Allergies:
The child’s safety is of upmost importance to us and is our first priority. It’s the registered Parent or Guardian’s responsibility to inform the Center of any food or other allergies when completing the enrollment documentations and also through open communication during our interview process.
Meals & Snacks:
The children will not personally receive any food from our Center in respect of each family’s eating habits and food choices as well as possibility of food allergies. You are permitted to bring your own food, please clearly mark the container with your Child’s name. For your children, food, milk and the formula shall be supplied by the Parent/Guardian. Children will be encouraged to eat what is served, but are not forced. Meals and snacks will be served as scheduled times;
Morning Snack: 10:00 a.m.
Lunch: 12:30 p.m.
Afternoon snack: 2:30 p.m.
If your Child has a special diet or allergies or there’s a special occasion, please bring meals and snacks prepared from home in order for us to serve to them.
Supplies:
Parent(s)/Guardian(s) are responsible for supplying diapers, wipes and creams for their child. Please write your child’s name on any supplies provided.
Upon enrollment and every six months afterwards, each family must contribute water, on perishable food items and a full set of clothing for their child to be stored with the emergency supplies in case of earthquake or other natural disaster, or emergencies.
Change of Clothing:
Parents are responsible for maintaining a spare set of clothing in their child’s cubbies in a plastic bag with their names on it. Please check frequently to make sure the spare set of clothing you provide matches your child’s current size.
Emergency Preparedness:
We have Comprehensive Emergency Checklists in place, including; Pediatric First Aid,

Absences:
The Parent(s)/Guardian(s) must notify us no later than your regular drop off time if:
– Your child is sick and will stay home.
– For any other reason won’t be attending the day care that day.
Vacations:
Our contract is to ensure a “slot” for your child at our daycare. For that reason, absence for any reason, does not alter the basic terms of our agreement. Therefore, the Parent/Guardian is responsible for paying the weekly rate, irrespective of actual attendance. Absence due to illness, holiday or vacation days, will not alter the regular weekly rate and must be paid in advance. Notice of vacation absences should be given at least two weeks in advance including payment. Five (5) consecutive days of absence without notice or payment will automatically terminate our contract. Full fee will be charged for vacation time, including holidays and any absences. As much notice as possible will be given when the provider schedules a vacation time.
Holidays:
Our family childcare will be closed on the following PAID Public holidays. Please arrange for alternative care on these days. New years day, Martin Luther king day, President day, Passover, Memorial day, Independence day, Labor day, Veterans day, Rosh hashanah, Yomkippur, Thanksgiving day and Friday after it, Christmas eve, Christmas day, Columbus day, Winter & Summer vacation TBD – with Parents being notified ahead of time.
Illness of Child:
The daycare Center is not authorized to care for sick children.
The Parent/Guardian is responsible for providing an alternate daycare plan in the event of child’s illness.
If the child is ill when he/she arrives at our facility, it will be determined at the sole discretion of the Center as to whether the child will be allowed to stay.
If child becomes ill while under the care of the Provider, the Parents will be notified immediately and pick-up of the child will be expected.
A child will not be re-admitted to the daycare after a serious or contagious illness without a written statement from the child’s Physician certifying that he/she is no longer contagious.
The daycare Center shall not administer prescription(s) or over the counter (OTC) to a child without Parent/Guardian authorization.
If your child has any of the following illnesses or conditions, you must keep your child home:
If it is clear that your child is just not feeling good.
Depending upon the illness, the Parent or Guardian may be required to obtain a written note before the child can return to daycare.
A temperature over 100°F,
Vomiting (2 or more times in 24 hours),
Diarrhea (3 or more watery stools in 24 hours),
A rash or nits,
Eye infection,
Sore throat,
Any communicable disease.
Medications:
Any medication must be in the original bottle or container and prescription medication must include the original prescription label and instructions.
Our Center will not administer any medication, cream or sunscreen without written consent from the child’s Parent/Guardian. Medication consent forms are available on the bulletin board above the sign-in sheet. Please do not leave medication on any counter.
Parent/Guardian must hand all medications, instructions and consent forms to the Manager personally.
Immunizations:
Complete immunization records must be on file prior to your child’s first day of enrollment.
Withdraw of the Child:
The registered Parent/Guardian may terminate this agreement by issuing a written termination notice two weeks in advance or immediately with the payment of two (2) weeks fee.
Termination:
A minimum of two (2) weeks advance notice shall be given by all parties in case of termination. If Parent/Guardian wishes to remove their child from the Center before the two-week period is up, Parent/Guardian is responsible for paying for the full two-week period. This agreement may be adjusted or modified after one year.
Cancellation by Provider:
The Center reserves the right to terminate the contract at any time.

Please contact us for the current rates and methods of payment.
Discipline Policy:
Corporal punishment (i.e. spanking or hitting) IS NOT ALLOWED AND WILL NOT BE USED.
Statement of Non-Discrimination:

Modification & Amendments:
The Center reserves the right to amend or modify this agreement upon two (2) weeks written notice. Changes become valid upon notice being signed by all parties.
Waiver:
The waiver by the Center of any breach of the terms of this agreement shall not constitute a continuing waiver or a waiver of any subsequent breach of the same or different terms of this agreement. Any waiver must be provided in writing.
Government Law:
Our agreement shall be governed by and interpreted in accordance with the laws of the State of California.
